Payment is required to confirm your registration.
- If you have the names of your team members and you wish to register online with payment by credit card, complete the registration form below and proceed to secure payment via PayPal.
- Otherwise, complete the attached Excel form and return it via email to the address indicated or by fax to 514 843-8255 and proceed with your payment prior to the team registration deadline: February 1st, 2012.

IMPORTANT: registration is $250 per person.
For every ticket sold, a receipt for income tax purposes in the amount of $125 will be issued to the company identified on the registration form for the donation portion of each registration.
Registration fees are non-refundable.
